A seperate form will need to be filled out for each child. If filling out a form for multiple children, Please ensure the Parent/Guardian name is filled in on each form, however you do not need to fill in the other parent information unless the information is different for the other children.

For the safety and security of every child, all children must be dropped off and picked up by an adult. Children will only be released to individuals listed on the registration form as an authorized pickup or alternative pickup person. If the individual picking up the child is not listed, the child will not be released unless the church has been contacted in advance due to an emergency and approval has been given.

These policies are in place to ensure clear communication, proper documentation, and the highest level of safety for every child in our care, which is a standard practice in childcare and church ministry environments.

When the form requests contact information, please provide the most accurate and up-to-date information possible. This helps us communicate with you quickly if there is a need to reach out regarding your child or if there are any changes, updates, or important announcements related to VBS.
